Registration Terms & Waiver

By registering a participant for any program, clinic, camp, league, or event offered by The Factory Basketball Academy, you acknowledge that you have read, understood, and agree to the following terms on behalf of yourself and your child.

Request for Permission

I, the above-referenced youth athlete's parent or legal guardian, hereby register my child to participate in programs, activities, clinics, camps, leagues, and events organized by The Factory Basketball Academy.

Assumption of Risk

I acknowledge and understand that participation in basketball and related athletic activities involves inherent risks of injury.

I understand that my child will participate under the supervision and direction of coaches and/or volunteers. I agree to encourage my child to follow the rules of the sport and the instructions provided by coaches and staff to help reduce the risk of injury to themselves and others.

Despite reasonable precautions, I acknowledge that neither The Factory Basketball Academy nor its coaches, volunteers, officials, directors, employees, or representatives can eliminate all risks associated with participation. Injuries may occur and may range from minor injuries to serious injury, permanent disability, or death.

I knowingly and voluntarily accept and assume all risks associated with my child's participation.

Release and Waiver of Liability

In consideration of allowing my child to participate in programs offered by The Factory Basketball Academy, I agree, on behalf of myself, my child, and any other parent or legal guardian, to release, waive, discharge, covenant not to sue, hold harmless, and indemnify The Factory Basketball Academy and its coaches, volunteers, officials, directors, officers, members, employees, agents, and representatives from any and all claims, demands, actions, damages, losses, or causes of action arising from or related to my child's participation, including claims arising from injury to person or property, whether known or unknown.

Photography and Media Consent

I understand that photographs and videos may occasionally be taken during Academy programs and events.

By completing registration, I consent to The Factory Basketball Academy using photographs or videos of my child for promotional purposes, including but not limited to the Academy's website, social media channels, printed materials, displays, brochures, newsletters, and other marketing materials, without compensation.

Medical Authorization

I certify that, to the best of my knowledge, my child is physically able to safely participate in the activities for which they have been registered.

I understand that, in the event of illness or injury, representatives of The Factory Basketball Academy will make reasonable efforts to contact me or the emergency contact listed on the registration form.

If neither I nor the emergency contact can be reached in a medical emergency, I authorize Academy officials, coaches, or designated representatives to obtain appropriate medical treatment for my child, including but not limited to first aid, emergency medical services, diagnostic testing, hospital care, or other necessary treatment.

I accept full financial responsibility for any medical treatment or related expenses incurred on behalf of my child.

Acknowledgement

By submitting a registration through The Factory Basketball Academy website, I acknowledge that I have carefully read this Registration Terms & Waiver, fully understand its contents, and voluntarily agree to be legally bound by its terms on behalf of myself and my child.
